Description:

Are you an experienced healthcare professional looking for a rewarding career in a supportive environment? Ontario Health atHome is seeking Care Coordinators to assess patient needs, provide referrals to community services, and engage with patients, caregivers, and other healthcare practitioners. You will play a key role in delivering patient-centered care and be supported by a collaborative team of professionals.

Key Responsibilities:

  • Assess care needs and determine eligibility for services
  • Link patients with service providers
  • Coordinate and monitor care plan delivery
  • Establish relationships with patients and their families
  • Balance patient needs with available resources

    • Qualifications:

  • Membership in good standing with the applicable regulatory body
  • 2+ years of recent experience in community health or related field
  • Knowledge of healthcare delivery system and community resources
  • Excellent interpersonal and communication skills
  • Effective time management and organizational skills
  • Valid driver's license and access to a reliable vehicle
  • Proficiency in a Windows environment
  • Mandatory COVID-19 vaccination policy
