What The Role Involves:

* Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
* Working with patients (both in-person and virtually) to provide exceptional nursing services
* Providing services in a variety of settings which may include in-home, clinic, retirement home or school
* Communicating and liaising with patients, family members and others within the interdisciplinary care team
* Performing other clinical duties as required

What You Bring:

* Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
* Current College of Nurses of Ontario (CNO) registration in good standing
* A strong desire and commitment to making a difference in the lives of our patients
* Exceptional interpersonal skills
* A valid driver's license and vehicle as regular travel is required
* Excellent English verbal and written communication skills
* CPR Certification
* Current immunization record including 2 step TB test
* To protect our patients, we require a current vulnerable sector check and a clear background check
