Hours: 37.5 hours biweekly (8am–4pm, weekdays)
Pay: $26.97–$49.63/hour
Travel Required: Hamilton, Burlington, Brantford, Niagara, Norfolk

Role Summary:
The Dietitian provides nutritional assessment and support to adult clients recently discharged from hospital who may have medical, mobility, mental health, or addiction needs. The focus is to improve nutritional health, support recovery, and promote independence while working closely with clients, families, and care teams.

Key Responsibilities:

Assess and monitor clients’ nutritional status

Develop personalized nutrition plans and provide dietary education

Participate in care conferences and document progress

Support staff with nutrition-related guidance

Connect clients with community food resources and services

Perform preliminary swallowing assessments

Maintain up-to-date knowledge of nutrition standards, especially for seniors

Requirements:

Bachelor’s degree in nutrition or related field

Completed dietitian internship

Registered and in good standing with the College of Dietitians

Experience with geriatric or community health preferred

Valid driver’s license and ability to travel regionally

Strong communication, teamwork, and problem-solving skills
