Southlake Health is seeking an Exercise Therapist for their Cardiac Rehabilitation program. The role functions as a Case Manager for a designated group of participants and acts as a liaison between participants, their primary care physician, and other healthcare providers. Responsibilities include ongoing assessment of participants’ progress, attitude, and behavior, and adjusting treatment plans in collaboration with medical and allied health team members. The therapist also assists in participant and family education and provides motivational support for long-term behavior modification.

This role requires strong knowledge of cardiovascular and exercise physiology, ECG interpretation, cardiovascular medications, and principles of behavior modification. The Exercise Therapist must be able to assess participants, plan individualized programs, and work effectively as part of an inter-professional team. Experience in adult education, program planning, research, and Microsoft Suite is also highly valued. Effective communication, problem-solving, leadership, and teamwork skills are essential.

Candidates should hold a BSc in Kinesiology, Physical Education, Physiotherapy, or a related field, with electives in cardiac and exercise physiology and cardiovascular rehabilitation. Eligibility for membership with the Cardiac Rehabilitation Network of Ontario and the Canadian Association of Cardiovascular Prevention & Rehabilitation is required. Preferred qualifications include cardiopulmonary exercise testing experience, current BCLS certification with AED, ACSM Clinical Exercise Specialist, and CSEP CEP certification, as well as a minimum of 2 years of relevant experience in cardiovascular rehabilitation.
