22.5 hours per week Calderdale Core pathway is a large pathway that includes Nurses, Social Workers, Support workers, psychologists, and Medical Staff. The Core pathway focuses on different types of service users, offering short-term interventions before discharge back to primary care.

As a health care support worker in the core pathway, responsibilities include assessing service user needs, holding a caseload, providing tailored support, completing outreach visits, working with service users to create care plans, and collaborating with GPs and other healthcare providers.

Main duties involve holding a caseload, providing tailored support, completing outreach visits, working with service users on care plans, helping with recovery, liaising with healthcare providers, and adhering to infection control procedures.

Qualifications required include GCSE in Maths and English, level 3 qualification in Health & Social Care, Care Certificate, experience in personal care, and working effectively in a team. Training, physical attributes, special knowledge & skills, and personal attributes are also essential for this role.

Desirable qualifications include level 4 units in Health & Social Care, experience in the specialty area, and willingness to undertake further study. Language skills other than English and supervisory experience are also desirable.
