Job Description:
Mackenzie Health is seeking a Nurse Practitioner (RN EC) for the Chronic Kidney Disease (CKD) Program. This role involves advanced nursing practice with an expanded scope, including diagnosing conditions, prescribing medications, and ordering diagnostic tests. The NP will manage patient care, develop individualized care plans, and provide health education while ensuring safety and quality in a fast-paced environment.

The successful candidate will exercise professional judgment using best practice guidelines, contribute to a safe and inclusive workplace, and provide patient- and family-centered care. Responsibilities include chronic condition management, disease prevention, health promotion, rehabilitation, and continuity of care. Strong organizational, analytical, and communication skills are essential, along with proficiency in computer systems.

Candidates must be registered with the College of Nurses of Ontario as an Extended Class RN, hold or be pursuing national certification in Nephrology Nursing, and have current BCLS certification. Preferred qualifications include at least five years of clinical nephrology experience, case management skills, and interest in acute and chronic hemodialysis care.
