Job Description:
Mackenzie Health is seeking a Nurse Practitioner to join the Nurse-Led Outreach Team (NLOT), providing advanced clinical care to residents in long-term care homes within the Mackenzie Health catchment. The role involves assessment, diagnosis, treatment, education, consultation, and research in geriatrics, aiming to reduce avoidable ED transfers and hospital admissions while promoting a resident-centered care model.

Responsibilities include collaborating with healthcare team members, advocating for clients, and ensuring safe, inclusive, and high-quality care. The NP will contribute to improved system partnerships, increased capacity, and enhanced information exchange to optimize patient outcomes across organizational and system levels.

Candidates must hold a Master’s Degree in Nursing and current RN (EC) Nurse Practitioner certification (PHCNP or NP-Adult) with at least two years of experience in adult medical care. Preferred qualifications include specialty certifications in emergency or gerontology nursing, membership in professional nursing associations, and the ability to provide both on-site and virtual consultations.
