Description:
The Pharmacist at Toronto Western Hospital will provide comprehensive inpatient pharmacy services as an integral member of the patient care team. This role involves conducting thorough medication histories, performing medication reconciliation, reviewing doctors’ orders for appropriateness, and identifying, resolving, and preventing drug therapy problems. The Pharmacist will monitor drug therapy for effectiveness and safety, ensuring adherence to evidence-based medicine and guidelines.

In addition to direct patient care, the Pharmacist will provide education to patients regarding their medications, facilitate discharge reconciliation, and respond to drug information queries. They will also provide supervision to pharmacy technicians and support clinical teaching and research activities within the department. This role requires collaboration with multi-disciplinary teams to optimize patient outcomes and uphold UHN’s values of safety, compassion, teamwork, integrity, and stewardship.

Ideal candidates will hold a Bachelor of Science in Pharmacy or PharmD, possess active Ontario College of Pharmacists registration, and have hospital pharmacy experience with patient-centered care. Excellent organizational, communication, and interpersonal skills are required, and additional experience such as hospital pharmacy residency or membership in professional societies like CSHP is considered an asset.
